How to Refer a Patient

Referrals can be submitted by fax, secure email, or phone. Include the patient's diagnosis, relevant diagnostic results (sleep study or oximetry), and contact information. We will contact the patient within [1-2] business days.

What to Include in a CPAP Referral

  • Completed sleep study report (polysomnography or home sleep test)
  • Diagnosis and recommended PAP therapy type
  • Patient's health card number and contact information
  • ADP physician form (if initiating ADP funding)


What to Include in an Oxygen Referral

  • Blood gas or oximetry results meeting ADP criteria
  • Diagnosis and prescribed flow rate / delivery mode
  • ADP Form O (Oxygen Prescription) signed by physician
  • Patient contact information and home address
